Participation Consent

Please read carefully before agreeing:

Beauty Kulture Academy is a licensed beauty education and training facility that provides hands-on learning experiences in areas such as skincare, hair, nails, makeup, and beauty business development.

During Teen Camp, participants will engage in supervised, age-appropriate activities that may include:

• Basic skincare treatments and product application
• Introductory hairstyling techniques
• Nail care and polish application
• Makeup application and beauty techniques
• Interactive learning, demonstrations, and hands-on practice

All activities are conducted in a controlled, educational environment under the supervision of trained instructors. Beauty Kulture Academy follows professional standards related to sanitation, safety, and product usage, similar to those taught in licensed cosmetology and esthetics programs.

Acknowledgment of Risk

I understand that participation in beauty-related activities may involve minor risks, including but not limited to:

• Skin sensitivity or allergic reactions to products
• Minor irritation from skincare or cosmetic application
• Accidental spills or minor discomfort during services

I confirm that I have disclosed all known allergies, sensitivities, and medical conditions that may affect my child’s participation.

Parent/Guardian Consent

• I have read and understand the nature of the activities offered at Beauty Kulture Teen Camp
• I give permission for my child to participate in all supervised camp activities
• I understand that my child will be participating in hands-on beauty services in an educational setting
• I accept the inherent minor risks associated with participation
